Bacteriostatic versus sterile water: what the preservative changes

Sterile Water for Injection is water with nothing added. Bacteriostatic Water for Injection is the same water with an antimicrobial preservative, benzyl alcohol, added to it.
The preservative is what makes a container intended for more than one withdrawal. It inhibits the growth of bacteria that may be introduced when the closure is entered. That is its documented job.
It does not make the container sterile after entry, does not sterilise anything added to it, and does not stabilise or preserve whatever the water is mixed with. Those are separate properties.
Benzyl alcohol carries its own labelled warnings, including a documented concern in newborns. That is a statement on the label, not an inference about the ingredient.
This page compares two labelled products. It states no volume, no duration, no route and no procedure, and it does not tell you which product applies to any situation — the labelling of the specific product and a licensed pharmacist do that, with the container in front of them.
Two products, one ingredient apart
Bacteriostatic Water for Injection, USP is sterile water to which an antimicrobial preservative — benzyl alcohol — has been added, and its labelling describes it as intended for preparations requiring more than one withdrawal from the same container. Sterile Water for Injection, USP is the same water without that addition, and its labelling says so explicitly: it contains no bacteriostatic or antimicrobial agent.12
That is the whole of the chemical difference, and it is worth stating plainly because most of what circulates about these two products is not about the preservative at all. It is about durations, volumes and stability, none of which is a property the preservative confers.
What the preservative does
The word to read carefully is bacteriostatic. A bacteriostatic agent inhibits bacterial growth; a bactericidal one kills. The preservative is present so that organisms introduced into the container when its closure is entered do not multiply in what remains. It is a control on what happens after contamination, not a guarantee that contamination does not occur.1
That is why the concept exists at all in multi-dose presentations generally. Approved multi-dose injectables commonly contain an added preservative for the same reason — the labelling for insulin human injection, for instance, lists metacresol among its ingredients — and the WHO multi-dose vial policy exists precisely because a container entered repeatedly is a different infection-control object from one entered once.57
None of this makes the container sterile. WHO's injection-safety work is built on the observation that unsafe injection practice is a documented cause of transmitted infection, and the practices at issue are about how a container is entered and what enters it. A preservative does not supervise technique.6
What the unpreserved product's label warns about
It is easy to read Sterile Water for Injection as the safer, plainer option because it contains less. Its own labelling says otherwise, in a specific way: the product is hypotonic and hemolytic, and labelling directs that it not be injected until made approximately isotonic by the addition of an appropriate solute. Several presentations are labelled as pharmacy bulk packages for compounding and not for direct infusion.2
That is the shape of the comparison. Neither product is the cautious default. They are two differently labelled things, each with statements that apply to it, and reading only the ingredient list of either misses the part that matters.12
The benzyl alcohol warning is a label statement
Preserved products carry warnings about the preservative. Benzyl alcohol has a documented association with serious and fatal reactions in premature neonates and infants who received benzyl-alcohol-preserved drugs, described in FDA's own review material and reflected in labelling across products that contain it.3
The general point generalises further than the specific one. An added excipient is an added substance with its own toxicology, its own population-specific warnings, and its own place in a label's warnings section. "It is only a preservative" is not a pharmacological argument; it is a way of not reading the label.13
What neither product tells you
The most common error is to treat the choice of diluent as a stability statement about the resulting preparation. It is not. A preservative acts on microbial growth. Chemical degradation of a peptide — deamidation, oxidation, hydrolysis, aggregation — proceeds by its own routes and is not addressed by an antimicrobial agent at all.
Nor does either label produce a beyond-use date for something compounded at home. USP General Chapter <797> derives beyond-use dating for compounded sterile preparations from the classification of the compounding environment, the aseptic process used, personnel practices and, for some categories, sterility testing. Remove those inputs and the standard yields no figure — not a longer one, not a shorter one.4
The part that belongs in a record
What a records site can contribute here is narrow and boring. Whichever diluent a container was drawn from, the record should identify it well enough to trace back to its own label: product name, manufacturer, lot number, and the date the container was first entered. A diluent is a source of material like any other, and an unidentified one is an unresolvable field in the record.1
Buying a particular brand proves nothing about a preparation and extends nothing. The only claim made for the link above is that a container has a label and a lot number on it, and that those belong in the record.
Is bacteriostatic water sterile?
The product as supplied is sterile water with an added preservative. The preservative inhibits growth of organisms introduced when the closure is entered; it does not restore sterility to an entered container and does not sterilise anything mixed into it. Sterility after entry is a function of technique and environment, not of an ingredient.
Which one should I use?
This page will not answer that. Which diluent applies to any preparation is stated in the labelling of the specific product and decided by the pharmacist or clinician handling it. A general article that named one would be wrong for most readers.
Does bacteriostatic water make a peptide last longer?
No claim of that kind is supported here. The preservative acts on microbial growth. Chemical degradation pathways — deamidation, oxidation, hydrolysis, aggregation — are unaffected by an antimicrobial agent, and no published study has measured potency over time for research-grade peptides in either diluent under home conditions.
